Reproductive and Perinatal Mental Health Services

Dr. O’Brien has specialized expertise in reproductive and perinatal mental health. She has completed advanced training through the American Society for Reproductive Medicine’s Mental Health Professional Group and earned certification as a Perinatal Mental Health Professional (PMH-C) from Postpartum Support International. She is well-equipped to support individuals and families through fertility-related stress, pregnancy and postpartum transitions, perinatal loss, and the emotional complexities of reproductive decision-making.

Third-Party Reproduction Evaluations

Third-party reproduction opens meaningful paths to parenthood for a diverse range of individuals and families. Dr. O’Brien works with intended parents, donors, and gestational carriers with thoughtfulness and care, providing evaluations that meet the highest clinical standards. Her evaluations follow guidelines established by the American Society for Reproductive Medicine (ASRM), utilizing both a structured clinical interview and the Personality Assessment Inventory (PAI). This dual approach ensures a comprehensive understanding of each participant’s readiness and well-being. Evaluation fees are based on Dr. O’Brien’s hourly rate and the complexity of the case, which may be discussed during the consultation call.

Intended Parent (IP) Counseling Sessions

Recipient counseling provides intended parents the opportunity to reflect, ask questions, and prepare thoughtfully for their path to parenthood using donor gametes or embryos. Most sessions last approximately 75 minutes and are guided by ASRM guidelines to:

  • Explore emotions related to the absence of a genetic link

  • Consider readiness and motivation for using a donor

  • Discuss plans for disclosure to children, family, and others

  • Review donor selection and embryo disposition issues

  • Address any specific concerns for single parents and LGBTQ+ families

Donor Psychological Evaluations

Dr. O’Brien provides psychological evaluations for egg, sperm, and embryo donors. These include:

  • Structured clinical interview covering psychological history, functioning, and informed consent

  • Administration of the PAI

  • A formal report submitted to the referring agency, fertility clinic, or reproductive attorney​

Gestational Carrier (GC) and Surrogate Evaluations

These evaluations are comprehensive and align with ASRM recommendations. Sessions address:

  • Psychological and emotional readiness

  • Motivations for carrying

  • Previous pregnancy and parenting experiences

  • Support systems and relational dynamics

  • Ability to separate from the child post-birth

  • Partner’s role and perspective (partners are included in the process)

Group Interview Sessions

A 30-minute group session with the intended parents, gestational carrier, and partner(s) provides a space for open dialogue and shared expectations. Topics may include:

  • Communication preferences

  • Prenatal care and birth planning

  • Post-birth relationship

  • Conflict resolution strategies

These personalized, client-driven consultations are perfect for those who may not be ready to commit to therapy but would benefit from checking in with a certified perinatal mental health provider. This appointment type might be right for you if you are interested in creating a birth and/or perinatal wellness plan, assessing and managing risk for perinatal mood disorders, or checking in at various stages of your reproductive journey for education and support.
